it's usually result of a funny discussion or event, when someone just states that somebody deserves a title...

Gerrie, if there's anything you'd like to get - just let me know. If there's someone who deserves a custom title, it surely is you.
If there's nothing like this, just wait for the right moment when the custom-title-inquisition strikes without warning
